One monthly fee for your everyday care.
Direct primary care is a different health care model allowing for more time and root cause analysis to get to the bottom of health issues. Instead of billing insurance for each visit, you pay us a set monthly fee, and your everyday care with Cynthia is covered: visits, sick visits, basic in-house labs and minor procedures.
It starts with a first visit of about 75 minutes: full labs, a physical, an EKG and a full review of your medications, supplements and medical and surgical history. It isn’t health insurance, so most people keep a plan for specialists, emergencies and hospital stays.

Questions about direct primary care
Answers to common questions. Anything else, ask us at your first visit.
Still deciding?
Call the front desk and we’ll help you decide if membership makes sense for you.
What is direct primary care?
It’s a monthly membership for your everyday care. Instead of billing insurance for each visit, you pay us a set fee, and it covers unlimited visits with Cynthia Diaz, FNP-C, sick visits, basic in-house labs and minor procedures.
How much does it cost?
$125 a month for one person, $200 for a couple, and $250 for a family of two adults and two children 18 and under. Not a member? An office visit is $175, and a telehealth, sick or refill visit is $75.
Is there a minimum commitment?
Yes. Memberships have a 6-month minimum, which gives us time to get to know you, review your labs and build a care plan that works for you. After the first 6 months, you can cancel at any time with one month’s notice.
What happens at the first visit?
It takes about 75 minutes and includes full labs, a physical, an EKG and a full review of your current medications, supplements and medical and surgical history.
Is direct primary care the same as insurance?
No. It covers your everyday care with us. Most people keep a health insurance plan for specialists, emergencies and hospital stays. Direct primary care works well alongside a high-deductible plan, and it’s also an option if you don’t have insurance.
Can I have a telehealth visit?
Yes, for things like prescription refills, lab reviews and follow-ups for ongoing conditions. Cynthia will tell you when a visit needs to be in person. You’ll need to be in Arizona at the time of your telehealth visit.
Can my family join?
Yes. The family membership covers two adults and two children 18 and under for $250 a month. If you have more children, contact us for pricing.
Do members get discounts on other services?
Yes. Members get 10% off our other programs and in-office services, such as our weight loss program, hormone replacement therapy and neurotoxin treatments.
Can I use my HSA?
Our monthly fees are within the IRS limits for direct primary care under the 2026 rules ($150 a month for one person, $300 for more than one), but whether a membership qualifies depends on other rules too. Check with your HSA administrator or a tax advisor.
What if I have an emergency?
Call 911 or go to the nearest emergency room. Direct primary care isn’t emergency care.
